How to Apply for Services at Dale Brain Injury Services

SOUTH WEST LHIN (SWLHIN) CARE COORDINATORS

SWLHIN Care Coordinators can make referrals through Health Partner Gateway (HPG) using the SWLHIN Referral forms. If possible, please include medical verification of the brain injury/stroke as this assists us to move toward an offer of services more quickly. If this is not possible we will work with the applicant to obtain this information.

WHAT TO EXPECT:

We will determine suitability for our services by reviewing the document verifying brain injury/stroke. If you have not been able to provide this information we will contact the client to obtain consent to obtain the necessary medical records. We may place the client on a wait list, depending on which services they are applying for. The next step will be a needs assessment, in which we will meet with the client, and their care partners and professionals if applicable, to determine which of our services best meet their needs and to offer services as soon as we have a space available.

HOSPITALS

SWLHIN Care Coordinators associated with your hospital can make a referral, as indicated in the SWLHIN Care Coordinator section.  Alternatively, please submit an ‘Application for Service’ by filling out this link or by faxing it to:  (519) 668-6783 or emailing it to admissions@daleservices.on.ca.  If able to send medical records along with the application, it will speed up the process, if unable to do so we will work with the client to obtain consent and request medical records.

WHAT TO EXPECT:  

The Application for your client will be reviewed and services offered as soon as possible if our services are required in order to assist your client to transition from hospital to home successfully. We will contact the referral source as well as the applicant to set up a needs assessment in order to identify which of our services would mostly likely benefit the applicant and will work with the applicant and your team to organize services. Your client may be placed on a wait list if services are not required or available immediately.

We are members of the Community Support Services Networks that exists throughout the SWLHIN. Each of the areas listed below has a phone number that you can call to apply for any Community Services that may be beneficial to you. Dale Brain Injury Services’ Programs are only one of the services that are available and we want to make sure that you are informed of all of the services that may benefit you.  Please phone the number for the county that you live in to hear more about these services.

WHAT YOU CAN EXPECT:

The person who answers the phone will ask you some questions in order to learn about what you are looking for. A referral will be made to services that are of interest to you and that will most likely benefit you. If no one is available to answer the phone please leave a message and your call will be returned. When we receive your referral, Dale Brain Injury Services will contact you to obtain consent to obtain your medical records that verify that you have experienced a brain injury/stroke. We may place you on a wait list, depending on which services you are applying for. The next step will be to complete a ‘needs assessment’, in which we will meet with you and your care partners or professionals if you wish, to determine which of our services best meet your needs and to offer services as soon as we have a space available.
