Available across the South West (Elgin, Oxford, London/Middlesex, Huron, Perth, Grey and Bruce Counties) , Group Services (GS) are flexible and provided in a group setting focusing on individual and group goal achievement, increased independence and quality of life. Groups provided offer social, recreational, wellness, skill building, exercise and therapeutic activities. Services are available on a short-term, transitional, or lifelong basis.
The Nigel and Rhonda Gilby Centre for New Tomorrows (CNT), located in London offers access to an Adult Day Program for individuals with ABI, as well as a Life After Stroke specialized Day Program in addition to a variety of group services 5 days/week with occasional evening and weekend activities.
Group Services Criteria:
- Meets at least one of the following:
- Potential to improve through the rehabilitation process
- Caregiver requires respite
- Requires support to maintain or increase quality of life
- Have identified rehabilitation or quality of life goals
- Has needs that can be addressed through group programming
- Demonstrated ability to learn and/or benefit from staff interventions
- Able to appropriately contribute, participate and benefit in a group services setting
- Medically stable
- Minimum 18 years of age, no upper age limit
Exclusion Criteria:
- High level of risk of harm to others
- Requires a two-person transfer
- Requires one-to-one support for activities other than eating and toileting
Discharge Criteria:
- Achievement of rehabilitation goals as identified in the Individualized Service Plan
- Low levels of participation (ongoing service planning and referrals arranged as needed)
- Repeated breach of service conditions as outlined in the Service Agreement
- Change in functional status where higher levels of support are required, exceeding available program staffing resources
- Are no longer willing or able to participate in therapy
- Have cognitive or health issues that interfere with participation
- Goals have been met and client ready to return to work or transitions to other community programs
