Job Description

We are currently seeking one permanent full time Weekend Night Rehabilitation Facilitator – Floater to work at multiple locations within the city of London. The successful candidate must be caring, energetic, resourceful, motivated and able to provide direct support to individuals living with disabilities.

Hours are from 9:00pm until 8:00am Friday through Sunday, 33 hours/week. The successful candidate will work overnight hours, but may be expected to work a variety of shifts at different locations that may include days, evenings, weekends and holidays. 

 Qualifications:

  • Degree or diploma in human services from a recognized institution or equivalent training and experience.
  • At least 18 months relevant experience in the human services field.
  • Knowledge of applied neurobehavioural model and rehabilitation issues as it relates to brain injury.
  • Ability to work in a dynamic and constantly changing work environment.
  • Capacity to work shifts that include overnight, evenings, weekends and holidays.
  • Proficient in the use of computers; Microsoft Office including Word, Excel, Outlook.
  • Ability to work independently as well as collaboratively.
  • Strong oral and written communication skills.
  • Ability to meet physical demands of supporting individuals with physical challenges.
  • Current valid G driver’s license, clean driving record, and access to own vehicle for transport.

Desired Qualifications:

  • Previous experience in direct support to persons with disabilities. 
  • Current in all mandatory training- First Aid & CPR, NVCI, Health & Safety, Person Centered Supports, etc.

Additional Responsibilities:

  • Provides direct support to clients in a variety of settings according to established and accepted standards of care and support.
  • Provides personal care support as required.
  • Effectively implements Client Service Plans & Support Plans.
  • Reads all relevant agency documentation in a prompt and timely manner.
  • Participates in maintaining a safe and orderly workplace.
  • Performs duties specific to overnight shifts.
  • Participates in DBIS standing or ad hoc committees as assigned.
  • Performs other duties relevant to the job as assigned.
